On Approval of the Form for Providing Information to the Central Executive Body Implementing State Tax Policy on Licenses Issued for the Use of Radio Frequency Spectrum (Radio Frequency Resource) of Ukraine for Each Taxpayer of Rental Payment for the Use of Radio Frequency Spectrum (Radio Frequency Resource) of Ukraine, Who Calculates the Amount of Rental Payment in Accordance with Item 11, Paragraph 3, Subsection 9-3, Section XX of the Tax Code of Ukraine

Order Approves New Form for Providing Tax Authorities with Information on Radio Frequency Spectrum Licenses of Ukraine. This concerns taxpayers of rental payments for radio frequency resource usage. The document consists of two parts: the order itself and the reporting form approved by it. The order cancels the previous version of the form dated 06.04.2021. The form contains the following main elements for completion:

– Data about the business entity (name, unified state register code, location)
– Information about licenses (registration number, issue and validity dates)
– Technical parameters of radio frequency usage (radio technology, regions of use, frequency bands, their width)
– Technological features of radio frequency band usage
